Start HIB 338 Form - To be Completed by Families/Caregivers

This digital HIB 338 Form should be used by parents or guardians to report allegations of HIB. Once the digital HIB 338 Form is received by the school, the principal is responsible for implementing the school district’s policy and procedures. An investigation shall be completed as soon as possible, but not later than 10 school days from the date of the written report of the incident. Should you have any questions about the investigation, please contact the school principal. Directions: Follow the prompts on the Hibster site to provide detailed information of the alleged HIB incident. If some fields are not applicable or if you are uncertain of the response, you may skip those fields. This form may be submitted anonymously.

To Be Completed by LEA Personnel (Teachers/Staff members/other school employees)

All allegations of HIB must be reported through HIBster and submitted to the principal within two school days from when a school employee or contracted service provider, witnesses or receives reliable information, that a student has been subject to HIB.

  • The submission of the digital HIB 338 form for reporting incidences of HIB is required.

  • This information shall be completed even if a preliminary determination is made under the LEA’s policy that the reported incident or complaint is a report outside the scope of the definition of HIB.

  • Required investigative procedures shall continue after the submission of this form to the school principal.

  • The completed form must be kept on file in the school.

  • The principal must promptly submit a copy of this form to the superintendent.

Millburn Schools expects all members of our community to follow our district’s Harassment, Intimidation, and Bullying (HIB) Policy. We encourage families to promptly report any concerns by completing the HIB 338 Form for Families/Caregivers. Once a report is submitted, the School Safety Team will review the information and respond in accordance with the New Jersey Anti-Bullying Bill of Rights Act.

HIB District and School Grade Report Cards
Each year, Millburn’s School Safety Teams complete a self-assessment as required by the New Jersey Department of Education. These assessments, based on the measures outlined in the Anti-Bullying Bill of Rights Act, determine the annual HIB grades for both the district and individual schools.
